Dual-Plane Augmentation Mammoplasty - Breast Implant

A breast implant is a prosthesis used to enlarge the size of a woman's breasts (known as breast augmentation) for cosmetic reasons, to reconstruct (e.g. after a mastectomy or to correct genetic deformities. Breast implants are medical devices with a solid silicone, rubber shell. The implant shell will be filled with elastic silicone gel. Silicone gel implants are much softer and give a very natural result.

If a silicone-gel filled implant leak or break, the elastic silicone gel may remain within the implant shell, or may escape into the breast implant pocket (a capsule of tissue that surrounds the implant). A leaking implant filled with silicone gel may not deflate and may not be noticeable except through imaging techniques such as an MRI. But today with the last generation of cohesive gell implants such a risk does not exist anymore. The new implants – some with life time warranty – do not leak even if they are cut with a sharp object. You may ask for life time warranty information conditions during your consultation.

The method of inserting and positioning your implant will depend on your anatomy and your surgeon's recommendation. The incision can be made either in the crease where the breast meets the chest or around the areola (the dark skin surrounding the nipple). Every effort will be made to assure that the incision is placed so resulting scars will be as inconspicuous as possible.

Working through the incision, the surgeon will lift your breast tissue and skin to create a pocket, either directly behind the breast tissue (sub-mammary or sub-glandular placement) or may be placed beneath the pectoral muscle and on top of the chest wall (sub-muscular placement).

Both pockets have advantages and disadvantages. A sub-mammary implant would lift the breast up perfectly in the lower pole – an advantage for saggy breasts and patients with little breast tissue – But would be visible in the upper pole, creating a siliconny decolteé. Sub-muscular implants lack this unnatural appearance due to the thicker muscle cover, but could not be effective in lifting the lower pole.

A new technique which is called dual-plane technique might be used in your case, with the implant placed sub-mammary in the lower pole for up-lifting, and sub-musculary in the upper pole for a natural decolteé. This technique fuses the advantages of the two older pockets and avoids their disadvantages.

Once the implant is positioned within this pocket, the incisions are closed with sutures, skin adhesive and/or surgical tape. A gauze bandage may be applied over your breasts to help with healing. The surgery usually takes one hour to complete.

Most of the times the patients are able to leave the hospital the same day of the surgery. You're likely to feel tired and sore for a few days following your surgery, but you'll be up and around in 24 hours. Most of your discomfort can be controlled by medication prescribed by your doctor.

Within few days, the gauze dressings, if you have them, will be removed, you will be allowed to take shower, and you will continue to use your surgical bra. You should wear it as directed by your surgeon. You may also experience a burning sensation in your nipples for about two weeks, but this will subside as bruising fades.

Your stitches will be absorbed on their own, but the swelling in your breasts may take three to five weeks to disappear.

Patients are given a list of detailed postoperative instructions, which include requirements for medications, hygiene, exercise and eating.

Breast Augmentation With Macrolane
